This form assists chapters in reporting their activities to the Psi Chi Headquarters for publication in Eye on Psi Chi magazine. If you have problems using this form, e-mail the Eye on Psi Chi editor HERE. By submitting your activities report, you are granting Psi Chi Headquarters permission to use any text and photos in our digital publications, limited print edition, website, social media, and any additional promotional materials.

Instructions

Up to three Activity Topics and three photos from each chapter may be selected for publication in each upcoming quarterly Eye on Psi Chi issue to inspire others around the world. Extra submissions may be “rolled over” to future issues at the discretion of the editor.

1. Under Activity 1, select an appropriate Topic for one of your chapter’s most momentous recent events (e.g., a specific recruitment event, a fund-raiser, or an induction ceremony).
2. Write 40 to 100 words in the third-person past tense to describe this specific event. You may also opt to complete Activity Topics 2 and 3 as desired.
3. Attach any available photos, and write a brief caption for each.
